The memorial, which includes "Outlines of a plan, proposed for the more effectual encouragement of vaccination in the United States" (p. 9-12) is dated Jan. 15, 1816
The "Appendix" (p. [13]-16) contains the text of a memorial to Congress by "certain citizens of Virginia and Pennsylvania," dated Jan. 15, 1813; a letter from Smith to Mr. Rhea, chairman of the Committee on the Post Office and Post Roads; and "An act to encourage vaccination," passed by the Congress and approved by President Madison Feb. 27, 1813
